Accurate information along with a patient photo for identification will help the doctor provide the best personal care, so we appreciate your time. You will also be asked to provide your credit or debit card through our secure payment processor for automatic monthly billing. Your membership will be confirmed upon receipt of your one-time enrollment fee (per family), but your monthly fee will not be charged until the clinic opens in September, 2017.
